When this form is completed it will be checked and verified by our team. For account customers we will have to complete this verification every 18months. For non-account customers we will have to verify every transaction for Regulated Substances. Verification will only take place during office hours 8am-5pm Mon-Fri excluding Bank Holidays. No Regulated Substances will be despatched until verification has taken place.
